Summary: To provide social work services to the participants of the Adult Daycare Program and their families/caregivers.
ESSENTIAL FUNCTIONS
Assesses social service needs of participants and/or family caregiver within five days of admissions and at least quarterly thereafter. Develops social service portion of participant care plan and adds progress notes at least quarterly.
Provides counseling to participants and family caregivers as needed to assist in coping with the stresses associated with caregiving.
Provides counseling to Center staff to assist them in coping with the stresses of caregiving cognitively impaired individuals.
Assists with discharge planning and assistance in transition.
Participates in care planning meetings.
Qualifications:
Valid Social Work License- MD
Bachelor''s degreeEstimated Salary: $20 to $28 per hour based on qualifications.
