Sante is seeking a Full-Time In-Home Intervention Therapist to join our team in Timonium, MD.
As an In-Home Intervention Therapist, you will be responsible for the assessment and in home treatment for clients in the community.
The ideal candidate will provide direct care, document all visits and interactions associated with the case, and will be the onsite resource for any relevant clinical information, as well as the liaison for collaboration with other involved parties including community providers.
What Youll Do Perform environmental and safety assessment upon entry to the community or residential setting where the visit is to take place as well as perform assessments for SI and develop safety plan if necessary.
Perform an assessment of client and diagnostic evaluation if appropriate.
Develop a treatment plan collaboratively with client based on identified client needs.
Provide individual, family and couples counseling, as needed.
Implement brief therapeutic strategies that are effective in reducing clients mental health symptoms.
Complete all documentation in a timely manner using EMR.
Demonstrate the capacity to work with a wide range of clients, child and adolescent, adult, dual diagnosis and geriatric.
What Youll Get Salary Range:
$31.
25 to $33.
65 per hour.
